How it works:
Patients may self schedule or health care providers can submit a referral.
Our Certified Wound Specialist APRN conducts the initial visit which includes a comprehensive interview, assessment and treatment of the wound, and individualized care planning — all clearly communicated with the patients care team.
We follow up weekly or as needed to provide advanced therapeutic interventions to increase the speed of healing and adjust the care plan as indicated.
The wound heals! When we discharge a patient, we remain just a phone call away if services are ever needed in the future.
